On 10/25/2016, at approximately 12:00 p.m., an employee was injured as the press crew began webbing the paper through the press rollers at the beginning of a press run. During the "web up," an excessive amount of paper built up in the area. The employee attempted to push the paper through the energized machine with her right hand resulting in the laceration and fracture of the middle finger.

An employee was trying to remove a blockage in a dust collection system. The airlock fan activated and amputated three fingertips on the employee's right hand.
