Final narrative
An employee was performing maintenance work on top of a tumble blast machine. As he was either climbing or descending a fixed ladder on the machine, he fell backward and landed on the floor below. He hit his head on steel framing on his way down. He suffered a head laceration, a fractured jaw, abrasions, and a contusion.
An employee was inside a sand muller machine using an air hose to clean out sand. The machine was activated and a metal mixer piece lacerated their upper thigh and fractured their femur. That machine was not locked out/tagged out.
An employee was repositioning a chill adder so that the pig tubs behind it could be removed. The employee lost their balance and their finger was pinched between the chill adder and the pig tub, resulting in a fingertip amputation.
