An employee was operating a stand-up forklift when he lost control while going down a sloped area. The forklift tipped over and his left foot was caught between the forklift and the curb, smashing his second toe, which had to be surgically amputated.

An employee was wrapping a pallet to be shipped. As he was walking around the pallet with a roll of shrink wrap, a forklift ran over his right heel and broke it.
