Final narrative
An employee was operating a grinder and lacerated the left inner thigh requiring hospitalization. The grinder was not guarded at the time of the incident.
An employee was unloading the paint line. Metal panel 1 was on a cart. Metal panel 2 was being lowered to release the hooks between the panels. The hooks twisted, became locked, and did not release, pushing panel 2 up and off the paint line hook. Panel 2 fell approximately 5 inches, amputating the employee's right-hand pinky fingertip.
An employee was placing a piece of metal in the press brake and caught his left ring and middle fingers in the press. Both fingertips were surgically amputated.
An employee was securing a lift hook to a table on a jib crane when the employee's right thumb tip was crushed/amputated between the hook and lift table.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
