An employee was operating a motorcycle on a trail when her wheels were caught in a rut on the side of the bike, causing the bike to overturn and land on her right leg. She broke her right tibia, fibula, and ankle.

An employee was operating a sit-down type of material handling equipment (MHE). They were turning the MHE with the clamp attachment in the air when the MHE fell over onto its right side. The employee's right foot was caught under the battery cover of the MHE. The employee sustained a chemical burn from the battery and a fractured right ankle.

An employee was driving a snowmobile when it flipped over. The employee suffered a broken rib.

Olympic Steel, Incorporated

An employee was driving an unloaded forklift outside a building. During a left turn, the forklift drove off the paved area onto an unpaved surface, became unstable, and tipped over. The employee was pinned under the forklift and suffered two broken leg bones and a broken hip.

An employee was riding in a utility task vehicle when it overturned. The employee sustained a broken leg.

West Tree Service, Inc.

An employee was trimming tree branches from an aerial lift. As they were maneuvering the aerial bucket lift from inside, the truck drove over soft ground and tipped over. The bucket lift fell to the ground with the employee tied off inside, resulting in rib fractures.

An employee was using a chainsaw to cut downed trees. The employee slipped and fell, and the chainsaw lacerated their left hand.

U.S. Department of Agriculture - Forest Service - Six Rivers National Forest

A crew of approximately 20 employees were constructing hand lines when a dead tree collapsed and injured three of the employees, including two hospitalizations. One employee was hospitalized due to shoulder injuries, and the other employee was hospitalized due to collapsed lungs, spinal injuries, and a broken tibia and fibula.

An employee was cutting brush and shrubs. A bee stung the employee on the base of their neck. The employee sustained an allergic reaction and was hospitalized.

An employee was engaged in initial smokejumper training which includes physical fitness testing, tree climbing training, chainsaw and crosscut certification, as well as initial introduction to equipment and smokejumper-specific safety procedures. Some of the training involves carrying 110 pounds of equipment. The employee reported tightness in his chest and difficulty breathing. The employee was hospitalized with a lung injury.

An employee participated in smokejumper training, which includes carrying weight. He suffered from dehydration and was hospitalized with rhabdomyolysis.

An employee was drilling a hole into the sealed hold of a vessel so a marine chemist could test its atmosphere. Upon drilling the hole, a gas that was under pressure was released and caught fire at the drill motor, where the employee was holding it. The employee was burned.

Navy NAVFAC-NW

An employee was performing restoration activities outside a 45-gallon condensation tank of a boiler system. As they were performing post-maintenance activities after repairing one of the two discharge pumps, one of the discharge valves would not open. The employee closed the intake pump to cool off the system and open the discharge valve. Two employees opened the inlet valve to return the tank to service. After they cracked open the inlet valve, a side wall of the tank blew out, causing the injured employee to suffer steam burns to half their body.

An employee was testing and troubleshooting the gang cylinder valve located in the gang area of the sawmill. The employee's fingertip was amputated by the gang cylinder valve.

SSA TERMINALS LLC

An employee was unlocking containers on deck. The employee was de-lashing while standing on the edge of the lid. The lashing bar he was holding came off the cone, causing him to turn 90 degrees and strike a railing behind him. The employee sustained four fractured ribs.

JONES STEVEDORING COMPANY

An employee parked a side-by-side on the fixed internal ramp aboard a cargo ship. They exited the side-by-side and went to inspect the cargo below. The side-by-side rolled toward the employee and struck them as they were walking toward the edge of the ramp. The employee was knocked off the ramp and landed on the lower deck approximately 8 feet below. The employee was hospitalized with a fractured shoulder.
