An employee was checking the back of a tractor-trailer for obstructions. The tractor-trailer driver lost sight of the employee while backing up and struck him. He suffered a broken leg, two broken ribs, and a road rash injury to the scalp.

An employee was winching in the mid-section of a boom on an aerial lift into the base section when their left middle and ring fingers were pinched between the two sections of the boom subassembly. The employee sustained a displaced fracture of the middle phalanx of their left middle finger and the amputation of their left ring fingertip.

An employee was preparing to use a programmable automated vertical band saw. The employee was clamping a piece of angle iron in place on the machine when the clamp caught the employee's left index fingertip. The employee suffered an amputation to the fingertip, up to the edge of the nail.

He connected the lifting hook to the steel part's center post, then applied tension to the overhead crane. His little finger was pinched between the lifting hook and the steel part, causing the amputation of the tip.
