While he was taking a fluke meter reading on the building's main switch gear (1600A, 480V 3 phase), an arc flash occurred and the fluke meter blew up in his hands. He suffered second degree burns to the hands, arms, neck, face, and eyes, requiring hospitalization. PPE was not worn at the time.

An employee was on an A-frame ladder pulling a wire through a conduit. The pressure caused the head to break from the fish tape, which caused the employee to lose his balance and fall approximately 5 feet, striking his head on the concrete. The employee sustained a brain bleed and injuries to the back of his head that required stitches.
On June 24, 2021, at 11:00 a.m., an employee was on a scissor lift 20 feet high splicing wires together from a panel to the rooftop units inside junction boxes. The employee was shocked, causing him to fall out of the scissor lift onto the ground and resulting in contusions and bruising that required hospitalization.
