An employee was positioning a piece of cardboard under a work table while two other employees held the table up when the table was dropped on top of her finger, which needed to be amputated.

An employee was performing equipment maintenance when he lost his balance and placed his hand down to stabilize himself. The employee's finger entered a nip point of a moving plunger, resulting in a fingertip avulsion that required amputation.

The injured employee was standing at the base of a ladder assisting another employee install a TV on a wall. The TV was dropped, striking the employee. They sustained a head injury.

Three employees were removing equipment from an old fermentation room at a bakery. They were using two bakery dollies to remove metal plates from the floor of the room. One of them was holding one of the metal dollies with his right hand on the side of the dolly, as a plate was being centered on the two dollies. The plate slipped and struck his right thumb, which was amputated near the last joint.

An employee was operating a drill and tap machine. He was placing a part with his right hand and removing a part coming in on the left side. He saw a part that was crooked and went to straighten it. The machine clamped down on his left index finger, degloving the top part of the finger and resulting in a partial amputation.

An employee was performing maintenance on a roll cutting machine in a warehouse. The employee was removing excess grease from the bearings when the machine's chain caught their right index finger. The fingertip was amputated by the chain and sprocket.

The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

General Thermodynamics, Inc.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
