An AECOM maintenance employee was troubleshooting a malfunctioning encoder device. To access the device, the employee had to stand on top of a tank holding a caustic cleaning solution, about 3 feet off the ground. The employee's left leg slipped through one of the access covers and was immersed in the caustic solution up to the knee. The employee suffered third-degree burns to the lower leg.

An employee was welding on a newly fabricated fuel tank when a flash fire occurred. Residual fumes in the tank ignited and flames exited the tank opening, causing burns to the employee's face and chest.

An aircraft mechanic was conducting maintenance on an aircraft from the ground. When he extended a boarding ladder, which is built into the aircraft, his left thumb was caught in pinch points and amputated from the nail to the knuckle.

While surveying in the middle of an intersection, an employee was struck by a vehicle and suffered broken ribs.

An employee fell when a rail broke, landing on the asphalt ground. The employee suffered fractures and was hospitalized.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was replacing the cutting edges on the bucket of a wheel loader. The bucket had been lowered onto composite blocking to prevent it from falling to the floor. The blocking dislodged and the bucket fell and crushed the employee's left hand.

An employee was walking on top of a piece of equipment while engaged in repair activities. The employee stepped onto a rotating impeller shaft of a belt-driven exhaust fan and sustained a partial amputation to their right leg below the knee and amputation of their left hand.

An employee was installing an automobile lift at a client location and put the lift on a safety lock. The employee's right hand was caught in the lift and crushed, causing the amputation of the little finger at the last knuckle and all of his right fingers were fractured.

On April 18, 2025, an employee was cutting the lid off an empty 55-gallon drum using a plasma cutter. The drum originally contained a flammable liquid and residual liquid inside the drum ignited and exploded. The employee was hospitalized with burns to the face, chest, and arms.

An employee was unlocking a yard gate from within a car. His finger was caught between the gate and the car, and the fingertip was amputated.

A driver was exiting his vehicle outside the plant gate when his foot slipped on the top step of the truck. He fell to the ground, landed on his left hip and elbow. The employee was hospitalized with a fractured hip/femur.

A security employee was responding to an urgent call from staff regarding a violent patient. The employee tripped and fell on the floor outside of the stairwell. The employee sustained a closed head injury, contusion of the cerebrum without loss of consciousness, and a closed fracture of the distal end of the right radius.

A security employee was holstering her firearm when the gun discharged a bullet into her right leg. The employee was hospitalized.

A temporary employee was testing an electrical starter motor. He was placing tape on the starter while the breaker was not engaged, but the starter sent an arc flash that burned his hands and stomach.

An employee was walking on a sidewalk and stubbed his toe on an elevated portion of concrete, causing him to trip and fall. The employee's right knee was dislocated.
