Final narrative
An employee was putting a cylinder in a deck when the cylinder would not set into the deck properly due to a misaligned bearing. He used a pry bar to raise the cylinder and realign the bearing. He was attempting to align the bearing cap when the pry bar slipped and the cylinder fell, causing the cylinder bearing cap to close on his left index finger. He suffered a left index finger partial distal phalanx amputation.
A machine operator was completing a job change at their machine when a cylinder roller (anilox) did not properly align in the machine. The operator went to the cylinder area to see what was keeping the cylinder from properly aligning. As the operator leaned toward the cylinder to inspect, the cylinder dropped into the correct cradle position. When this occurred, the operator's left hand was positioned between the cylinder and the cradle and their index fingertip was crushed, resulting in a partial amputation.
An employee was assisting a new employee with laminator machine startup. The extruder is on a carriage that can be moved along tracks in the floor. As the extruder was being moved into the machine, the injured employee stepped into the area to assist the new employee and then turned to move out of the area. As he turned, his foot was pinned between the extruder carriage and the floor stop, resulting in non-displaced fractures of the second and third metatarsals of the left foot. He was hospitalized.
Two employees were struck by the rotary table of a drilling rig while installing caisson. Employee 1 suffered multiple fractures, internal injuries, and head trauma. Employee 2 suffered multiple fractures, internal injuries, spinal injury, and head trauma.
Employees were felling hazard trees. The injured employee was conducting 'bucking' operations to saw a tree from the road when the tree kicked-back, pinning him against the ground and fracturing his left femur. The employee was hospitalized and required surgery.
An employee was chaining a vehicle when he had to change tools and the compressor. As the compressor was being charged, the employee stepped near the vehicle to make sure the compressor was attached. Then, he instructed a co-worker to lower the compressor. His right hand was on the compressor and under the chain, causing his ring finger to be crushed.

On October 25, 2025, an employee was stringing up a new reel of tire tread on a machine. The employee pulled a 6-inch tail and proceeded to spool up the liner. As the machine was jogged to begin the liner wrapping process, the liner caught the employee's glove and pulled their right arm into the liner, causing a fracture to the arm.
An employee was doing a cable change on top of an electric overhead traveling (EOT) crane. They were positioned between the cable drum and the drive shaft. While rotating the cable drum, the employee reached for the new cable that was to be installed.
