Final narrative
An employee was climbing a 15-foot, job-made wooden ladder to access a machine room when the ladder shifted. He then fell approximately 10 feet to the ground below, landing on his feet. He fractured his right heel, sprained his left ankle, and strained his back.
An employee and an inspector were performing an annual inspection on a hydraulic elevator. The employee slipped on ice and fell to the ground, fracturing their left hip.
The injured employee was part of a crew that was replacing an old elevator. The injured employee and a co-worker used a drywall dolly to take the old elevator base platform to a dumpster.
