An employee was making his rounds when he identified a level indication in the vessel that he was not sure was accurate. He then drained the sit glass and the material (mineral spirits polyethylene wax and ethaline gas) ignited a flash fire, burning his left palm, both forearms, and his left calf. He was hospitalized.

As the employee climbed to the fourth rung of the ladder, he struck the top of his hardhat on an overhead pipe.
