Kloeckner Metals Corporation

An employee was placing 4x4 timbers under a steel plate that was being loaded onto a trailer for delivery. The plate was raised about 12 inches by four chains with hooks on the four corners of the plate. One side of the plate was flat and the other side was raised; the middle section was beveled upward. While the employee was placing the timber under the plate with his left hand, the plate lost tension and fell, crushing the tip of their left index finger. The employee's fingertip was amputated.

An employee was transferring metal from a re-coiler to a turnstile. The employee's right middle finger was crushed between two coils of steel as he guided one of the coils off the processing line. He suffered an open displaced fracture to the finger.

An employee was moving a piece of an I-beam for welding when it rolled off the cribbing. The employee went to catch it when it fell and crushed the employee's finger, resulting in a partial amputation.

An employee was helping to disassemble large reels used to store steel wire when a 264-pound flange from the reel fell over onto the employee's left leg. The employee sustained multiple fractures to their leg and ligament injuries to the lower leg.

The employee's arm was fractured at the elbow and required hospitalization.
