Federal OSHA recorded a severe workplace injury at Lineage Logistics, 2088 Geneva Drive, GENEVA, ILLINOIS 60134 on — Fractures, affecting the lower leg(s).

An employee was operating a stand-up counterbalanced forklift and was attempting to push a stack of empty pallets using the rear, passenger compartment side of the forklift. As the pallets were being pushed, they became lodged between a rack and the forklift, causing the wooden pallets to break. Pieces of a broken pallet entered the passenger compartment of the forklift and struck the employee in the left leg, fracturing his tibia and fibula.

Employees were conducting inventory in the rail room. A forklift operator had moved four pallets of boxes into a storage location (bulk stacking) and pulled away. The injured employee entered this location to perform cycle counting/inventory and the stack of pallets fell forward, landing on the employee's right leg and back. The employee was hospitalized for a compound fracture to his right leg that required surgery.

An employee was operating a forklift in the loading dock area. They put a loaded pallet of product into a pallet invertor, then backed out of the pallet, and exited the forklift. Another forklift then struck the employee's left leg and pinned it against the employee's forklift. The employee's leg was lacerated.

An employee was using a dozer to clear a plot of land. The dozer struck the ground after getting stuck on a tree stump, causing the employee to hit their head on the cab roof. The employee sustained a spinal compression injury.

On August 31, 2023, an employee was operating an excavator to complete a demolition job. As the excavator drove into the first floor of the building, the floor collapsed. The vehicle and employee dropped approximately 8 feet, causing the employee to sustain a back injury.

The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
