When the jam was removed the lever that drops the feeder chains was activated and the feeder chains dropped on the injured employee's left hand. The injured employee sustained deep puncture/lacerations and a compression injury to the left hand.

While rotating the cable drum, the employee reached for the new cable that was to be installed. The employee was pinned between the drive shaft and two pieces of angle steel, resulting in a fracture to their left hip.
