As the employee was operating the overhead crane, the vibration caused the vertical shaker panel to fall over and strike the employee, fracturing the employee's left tibia.

He reached up to the edge of the railcar to catch himself while the ram of the hydraulic radar pusher was coming down. An employee was dumping a "self-dumping chip bin" (on the forks of a forklift) into a 3- or 4-foot-tall dumpster. The bin tilted forward. Thinking it was going to slide off the forks, the employee reached to grab the bottom of the bin to stop it. The bin then rotated back. The tip of the employee's left middle finger, from the first knuckle past his fingernail, was caught between the bin and the top of the forklift pocket frame. The fingertip was amputated just above the first knuckle.

An employee was operating a stand-up forklift in the warehouse when the forklift spun and the employee was caught between the forklift and metal shelving. The employee sustained multiple broken ribs.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.
