At about 4:11 p.m. on September 23, 2019, an employee was putting a rubber O-ring on a tire using a pry bar. The pry slipped and the wheel lock ring amputated the employee's left index fingertip.

An employee was inflating a farm tire and wheel assembly in a commercial tire restraint cage. The assembly failed and the cage absorbed the energy from the release of compressed air from within the cage, and the cage contacted the employee's arm. The employee was hospitalized with a fractured arm.

The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
