After repairing a piece of industrial equipment, an employee had brake-cleaning liquid on his gloves. A lighter ignited the liquid, and the employee's hands were severely burned.

On July 8, 2023, in the enclosure shop, an employee and two co-workers were setting a metal sheet onto the manually-operated lifting rail. As they tilted the lifting rail so that the metal sheet would move to the outside yard, the injured employee's right hand was caught between two moving parts of the lifting rail. His right middle finger was amputated at the distal joint and his right ring finger was lacerated.

On June 15, 2023, an employee was on a 6-foot A-frame ladder, installing a receptacle for a flat-screen television in an office. As he descended the ladder to retrieve a tool, he fell to the floor and sustained fractures to the right forearm/wrist.

An employee was guiding material into a punch press. The employee's left hand was caught by the press and his left index finger was amputated.

On January 27, 2023, an employee was taking the trash out to the dumpster located outside at the back of the building when she tripped and fell over the wheel blocks of the dumpster and onto the asphalt pavement. She sustained a left compound wrist fracture that required hospitalization.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee was sorting sucker rods. His right hand was caught in the area where the rod hits up against the frame and his index fingertip was amputated just below the nail.

An employee was using a powered miter saw to cut 2x4's. The saw blade hit something inside a board (possibly a hidden nail) and jumped, causing a laceration to the employee's left forearm. The employee was hospitalized with soft tissue and tendon injuries that required surgery.

A crane had been used to set a steel plate on a steel frame. An employee was trying to remove a clamp from the plate when the plate fell and crushed the tip of his left little finger. The employee sustained a fingertip amputation.

An employee was operating a scissor lift, raised to a height of about 15 feet, to inspect an overhead crane rail. The crane itself was being moved. Its hook swung into the scissor lift s arms, causing the lift to tip over onto its side, which in turn caused the lift and the employee to fall to the floor. The employee suffered injuries including broken ribs, spinal fractures, scapula fractures, and internal injuries.

A technician was operating a crane hoist to lift/remove a cylinder head from the main engine (port). The load was lifted at an angle, causing the cylinder head to get stuck on the cylinder head studs. When it became free from the studs, the cylinder head swung. It struck the technician and caught them against the cabinet door. This resulted in an amputation of their left index fingertip at the distal knuckle.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.

An employee was inspecting a generator whose radiator was leaking. He slipped, and a fan blade in the generator amputated his thumb and index finger.

An employee was operating an agricultural tractor during sugarcane harvesting. The employee sustained a lumbar sprain due to vibration or motion from the tractor.

An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
