An employee walked to the edge of a retaining wall to throw a chain rigging onto a flatbed truck. He stepped on the edge of the concrete block retaining wall, slipped, and lost his balance, falling approximately 5 feet and landing between the flatbed truck and the retaining wall. He fractured his right tibia.
An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.
