An employee was in the process of cleaning and setting a 20' steel siderail for a mold while standing on top of the 2.5' high mold. The employee stuck a piece of rebar into a hole of the mold to adjust the siderail's position in the mold. The employee pulled on the rebar and it came out of the hole, causing the employee to fall to the ground and fracture the right femur.

A crew was working to untangle a spool of 1/2-inch wire cable. One employee's left thumb became entangled in the cable, causing an amputation/avulsion. The employee was hospitalized.

An employee was setting a stress jack into place, getting ready to stress the cable. His hand was around the ram and his thumb was in the cylinder channel against the back wall. The stressing operator started moving the ram cylinder into position and the cylinder crushed the tip of the employee's left thumb against the back wall.

The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.
