Final narrative
An employee was performing maintenance work and replacing strips on a conveyor. The employee tripped and fell onto the conveyor hitting their arm. The employee was hospitalized for a fractured arm.
On December 17, 2020, an employee was standing on the fork of a forklift as it lowered a powered pallet jack. The jack tipped and the employee jumped to avoid being struck by it. He suffered a broken kneecap and an injury to the mid-leg.
An equipment delivery driver was lowering coolers using a lift gate. He lost his balance and fell backwards from the lift gate to the ground about 3 or 4 feet below, suffering lacerations to the back and front of his head. He was hospitalized.
