The cast deck crew were performing a ladle exchange, which involves the ladle turret being rotated from north to south to exchange the empty ladle with a full ladle. The crew were changing the ladle shroud during the ladle exchange. The crew removed the spent shroud from the empty ladle and turret rotation was then initiated to exchange the empty ladle. The crew was placing a new shroud onto the shrouding device, when an employee s foot was caught between the shrouding device and the access stairway on the north tundish car. The injured employee was hospitalized with a crushing injury to the left foot.
An employee was inside a motor/concrete pit of a roughing mill to put a cover on the motor. The employee reached to grab a ladder that was handed to him by a co-worker.
