An employee was inside a motor/concrete pit of a roughing mill to put a cover on the motor. The employee reached to grab a ladder that was handed to him by a co-worker. He had one foot on a 3x3 fiberglass grating cover that was in the middle of the pit when the grate slid, causing the employee to fall through. The employee sustained a back injury.
On July 7, 2021, at approximately 5:30 p.m., an employee was parking a front-end loader after moving hot coils from production to shipping in a steel mill. While exiting the front-end loader, the rear wheel on the left side of the loader exploded, throwing the employee approximately 20 feet from the boarding platform. The employee was hospitalized with broken bones in both ankles.
The cast deck crew were performing a ladle exchange, which involves the ladle turret being rotated from north to south to exchange the empty ladle with a full ladle. The crew were changing the ladle shroud during the ladle exchange. The crew removed the spent shroud from the empty ladle and turret rotation was then initiated to exchange the empty ladle. The crew was placing a new shroud onto the shrouding device, when an employee s foot was caught between the shrouding device and the access stairway on the north tundish car. The injured employee was hospitalized with a crushing injury to the left foot.
An employee was moving a 3-ton condensing unit, strapped down on a dolly, out of a garage. The strap broke, causing the employee to fall backward onto the brick pavered driveway. The employee suffered injury to a spinal ligament in the neck.
An employee was talking to a child on the playground, when the child grabbed her hand to lead her away. The employee stepped, lost her balance, and fell over. The employee was hospitalized with a fracture of her lower left leg and required surgery.
An employee was lowering a safe stack beam inside a trailer. The beam released, causing the employee to fall back and strike his head on the trailer floor. He was hospitalized with a brain bleed and a concussion.
An employee was on the ground working to close a trailer door. The door was caught by the wind and blew the employee backward. He landed on his back and sustained four fractured ribs, requiring hospitalization.
An employee used a crane to set down a bar. The bar measured 9 inches by 16 feet. The strap got stuck under the bar, and the employee asked another employee to jog the roll. The bar rolled and pinched the injured employee's left hand, resulting in a fingertip amputation.
An employee was loading a steel forming machine with steel to be processed. He went to move a cooling hose out of the way and his left thumb was crushed by the forming roller, resulting in injuries that required surgical amputation.
An employee was advancing a rail to line up for welding with a secondary rail on the weld line when his right middle and ring fingers were caught between the rails. The employee sustained an amputation to the top knuckle of the middle finger and a fracture to the ring finger. The machine was guarded at the time.
An employee was preparing bundles of 24-foot square metal tubing to be loaded onto a truck and cut the safety band for a bundle. While the employee was removing a piece of dunnage from a bundle, they bumped the bundle with their leg. The metal tubing then fell forward and struck the employee, causing a fractured left ankle.
An employee was changing a die in a press when the die slipped and crushed the employee's left index finger. The employee sustained an open facture of the tuft of the left distal phalanx and a partial amputation.
An employee was driving a boom lift (in the lowered position) in an exterior dock area. The lift s left wheels rolled onto base plates that covered a 3-foot-deep sump pit. The base plates failed, and one side of the lift dropped. The employee's left leg was caught under the lift basket, and he suffered a fracture to the lower leg including the ankle.
