An employee was cleaning a paving machine's hopper with a shovel. The employee noticed that the "auto" button was activated and tried to shut it off. The machine's front apron came down and caught the employee's right foot against the front bumper. He suffered a broken right foot, a broken left collarbone, and a burned right hand.

Two employees were using ATVs to help transport a 236-foot length of pipe. One ATV struck the other, causing it to roll over; its driver suffered broken legs, a broken pelvis, and a hand laceration. The driver was hospitalized. One of the driver's legs was amputated.
