An I-bolt (screwed into the heavy machinery and also being held by the hoist) broke, and the machinery dropped into the vat. Caustic splashed the employee's clothing, who suffered burns and was hospitalized.

An employee was working from an elevated platform, 35 feet high, while wearing a harness and retractable lanyard, to change out a gear on a paper machine. The employee suffered injuries including broken ribs, spinal fractures, scapula fractures, and internal injuries.
