Final narrative
An employee was pressing a bushing into an aircraft part using a rod bushing setting tool and a 5-ton arbor press mounted to a table. While the employee was pulling down the lever on the arbor press to seat the bushing into the aircraft part, the rod tool snapped, causing the employee to lose his balance and fall on his left knee. He suffered a broken left femur.
An employee was on top of a platform. The employee took a step back and fell off the platform and sustained a midshaft fracture of the right humerus with displacement and overriding of fracture fragments.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
