An employee was offloading propane from a truck into a large tank. When the employee removed a hose, a spark caused the propane to catch fire, resulting in second degree burns to the employee's face and neck.
An employee was monitoring a palletizer machine that stacks propane tanks when their shirt blew into the grippers on the palletizer. The employee used his right hand to brace himself and his hand was caught between the gripper and a propane tank resulting in a fracture, laceration and tendon injury to the right hand.
