An employee was sanding the end of a 1-inch-wide, 1/4-inch-thick, 12- to 13-inch-long rod on a pedestal grinder using a sanding disk attachment. The sanding disk caught the rod and pulled it toward the machine, causing the employee's gloved left hand to come into contact with the sanding disk. The employee's thumb tip was amputated to the cuticle.

An employee was disassembling a scrap headliner above a dashboard to recover automotive parts when their screwdriver slipped and contacted their right hand. The employee suffered a laceration that resulted in nerve damage, requiring surgery.

An employee was assisting in the removal of a filter dryer assembly from a support stand. The filter dryer was attached to an overhead crane during the time the mounting bolts were being removed from the support stand. Once the filter dryer was completely disconnected from the support stand, the unit became top-heavy and tipped over onto the employee causing a fracture to their right foot and lacerations to their upper right arm and upper back.

An employee was removing a 2,700-pound piece of metal from a horizontal mill. They were walking away from the machine when they slipped and tried to catch themselves on the metal, causing the large piece of metal to fall down onto their ankle. They sustained a compound fracture to their right leg.

An employee was standing on an elevated, unsecured platform to assist with the rigging and removal of a vessel from a work area when the platform tipped and they fell to the concrete floor, fracturing their pelvis and elbow.

An employee was repairing a CNC machine. A tool fell from the operator platform into the pit area below, a distance of approximately 3.5 feet. The employee descended into the pit using the access stairs to retrieve the tool. The employee's feet slid out from under him while stepping onto the cover, resulting in a fall and a fractured left hip that required hospitalization.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
