An employee was sanding the end of a 1-inch-wide, 1/4-inch-thick, 12- to 13-inch-long rod on a pedestal grinder using a sanding disk attachment. The sanding disk caught the rod and pulled it toward the machine, causing the employee's gloved left hand to come into contact with the sanding disk. The employee's thumb tip was amputated to the cuticle.
An employee was on an A-frame ladder retrieving materials from a shelf. He slipped and fell and his leg got caught in the ladder rungs, breaking his left tibia.
