An employee was stepping off a scissor lift onto a steel structure while tied off to a plumb line when they slipped and fell, resulting in a fractured left hip and elbow.

An employee was in the yard directing a forklift to move steel beams closer together on dunnage. The forklift boom was reaching over a stack of I-beams and while moving one of the stacks, the boom struck a lower level of dunnage and knocked over a 50-foot long I-beam weighing approximately 1,800 pounds. The I-beam struck the employee's legs from behind, breaking both legs.

An employee had been chaining a 6-foot long, 600-pound I-beam so it could be lifted. As it was lifted, the beam slipped from the chain and struck the employee, causing a fractured shin.

An employee was showing another employee how to shut off a stud machine when the cable on the stud machine arced. The employee reacted by jumping out of the back of the trailer (height of approximately 4.5 feet). His foot became caught on the ladder, causing him to land awkwardly on his arms. He suffered a fractured radial head on his left arm, as well as a broken radial head and dislocated elbow in his right arm.

An employee was cutting a length of steel coil when he noticed the steel might have cross-break defects. He used his gloved hand to feel for and clean the defected spots when his glove got caught in the measuring wheel, which rolls along the top of the steel to ensure accurate cuts. The employee's arm was pulled down into the machine until his body was stopped by a support bar that goes across the machine. The employee's arm was fractured at the elbow and required hospitalization.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.
