An employee was adjusting rolls of vinyl that were on a pole set horizontally on a rack. As the employee was pushing the vinyl with his right hand, the right side of the pole dropped about 3 feet, pulling the left side of the pole downward at an angle. The employee's left ring finger became trapped/pinched between the pole holding the vinyl and the rack that was holding the pole. The employee's fingertip was amputated.

An employee was performing maintenance on a press puller and aligning the puller when it fell. The puller electrical panel struck their left upper back, resulting in fractures to their T12 and L1 vertebrae. The employee was hospitalized.

An employee was conducting preventative maintenance on an extrusion press and using an impact wrench to remove rivets on the shear cylinder. When the last rivet was removed, the stored energy in the cylinder released and dropped it onto the employee's hand. Their right thumb was caught between the cylinder and the casing of the rivet gun, resulting in crushing injuries. The employee sustained an amputation of the thumb without bone loss. The machine was locked out/tagged out at the time.

An employee was broaching a keyway in a bore sprocket with a manually operated powered cylinder hydraulic press. On pressing the broach through the initial phase of cutting, he used an extension rod to finish broaching and clear the broach from the part. While setting up the extension rod, he placed his left index finger on top of the extension rod. The press extended and crushed his finger, resulting in an amputation.

An employee was standing in front of a forklift carrying several 2,000-pound "logs" of aluminum. The logs were banded together; when the employee cut the banding so they could be unloaded, one of them rolled onto the employee's foot. The employee's steel safety toe collapsed, and the employee's toes and metatarsals were crushed. Part of the foot was medically amputated.

An employee was performing finishing work on a residential driveway when they began to experience body cramps and were hospitalized for heat stress and dehydration.

An employee was staking tomato plants on a farm. The employee began to feel sick and collapsed, suffering from dehydration and heat shock.

An agent was participating in SWAT team selection and was performing various physical fitness skills including running and exercise intervals. The agent experienced dehydration and a muscular injury that required hospitalization.

An employee was traveling back from work when they became ill and experienced dizziness due to heat exhaustion.

On September 26, 2023, an employee was delivering packages when he began to feel ill with a pain in his side. He was hospitalized for heat exhaustion and dehydration.

A load of steel angles was being lowered when the lifting chain hit the load, causing it to slide toward an employee. An angle slid and pinched the employee, catching both of their legs between the beams and an angle. The employee suffered two fractured legs.

An employee was on the deck of a trailer helping a customer unload tubes. The employee was working to place a cargo strap on the remaining tubes using a winch bar when it slipped off the tubing, causing him to lose his balance and fall headfirst onto the asphalt lot below. The employee suffered a contusion on his forehead, resulting in hospitalization.

An employee and a customer were unloading two rectangular tubes. The tubes were wrapped together. The plastic wrap was cut so that one tube could be picked up. As the employee was lifting the top tube, the other tube (still connected to the top tube) came down onto his hand and broke two of his fingers. He was hospitalized.

An employee was assisting as a forklift got pipe from a pipe rack. The employee was on one of the forklift's forks as it was raised to the top of the rack, a height of about 15 feet. He fell to the ground and suffered a dislocated and fractured right elbow.

On September 23, 2025, at approximately 7:43 p.m., an employee was using a knife to cut poly banding from a metal coil. The knife slipped, causing a laceration to the inside of their left arm at the elbow joint. The employee was hospitalized and required surgery to repair a damaged vein.

An employee was moving a tensile strength test device with a dolly. The device fell from the dolly onto the employee's right leg, causing a compound fracture.

An employee was walking past a crossover conveyor when a ramp came down and hit them in the back. The employee sustained a broken back vertebra, as well as a concussion, and was hospitalized.

An employee was using a hook tool to pull a pallet onto the lift gate of a tractor trailer. The hook detached from the underside of the pallet, causing the employee to fall backward off the lift gate. The employee landed on the concrete parking lot about 5 feet below, suffering fractures to the skull and two thoracic vertebrae.

A skid loader backed over an employee's leg, breaking it.

An employee was walking on a truss table when he lost his balance and fell approximately 3 feet to the floor. The employee sustained a dislocated and fractured left ankle.
