The injured employee was guiding the pattern into the bolster as the crane lifted and lowered the pattern a few times to get it to seat. The injured employee then began checking for obstructions. The crane lowered the pattern onto the frame and the injured employee's right index fingertip was caught and amputated between the pattern and the bolster.

During this process, the sink caught on fire and the solvent bottle containing tert-butyl methyl ether fell into the burning sink.
