The rag caught on the roller and pulled his hand into the grinding wheel, resulting in a broken wrist, broken fingers, and lacerations to the hand and fingers. He was hospitalized.

An employee was clearing jammed material from an extruder machine. The employee reached their left hand through the unguarded opening between the extruder machine and the conveyor belt to manually dislodge the jam and contacted the moving chain and sprockets, resulting in an amputation to the left thumb.

At about 10:05 a.m. on March 10, 2025, an employee was working at the loading side of a mix conveyor when his right wrist was caught in a pinch point between the conveyor and a roller. He suffered laceration and fracture injuries to the wrist and was hospitalized.
