A cooling tower fan had stopped working. The breaker feeding the fan had been turned off, and an employee was investigating the problem with the fan. The subpanel was connected to a variable-speed motor; a circuit overloaded and an arc flash burned the employee's left arm, forearm, and hand.

An employee was installing a 15-amp breaker in an electrical panel when a 12-gauge ground wire touched the positive busbar of the panel, resulting in an arc flash. The employee sustained a second-degree burn to their left hand.

An employee was repairing an HVAC system in the drop tile ceiling of a conference room when they were shocked, causing them to fall from the ladder. The employee sustained burns to their right middle and ring fingers.

An employee was replacing a contactor inside an electrical panel attached directly to a press. The press itself was receiving power from another main panel. After replacing the contactor, the employee flipped the switch and an arc flash occurred, burning the employee's elbow, bicep, and neck.

An employee was attempting to switch a medium-voltage primary cable and install a 200-amp fuse barrel. The employee contacted the bottom of the switch gear cradle for the fuse barrel, causing an arc blast. The electricity entered the employee s left hand and exited his big toes, resulting in electric shock and burns to the left hand, arm, shoulder, and both feet. The employee was hospitalized.

On December 6, 2023, an employee of Duke Energy was working on a single-phase 120-/240-volt parallel service re-tap when a secondary flash occurred in an underground service. The employee suffered a second-degree burn to the face and was hospitalized.

ABC Compounding Co., Inc.

An employee was running a can coder that runs on a conveyor belt. The employee was stepping over the rollers when she slipped or tripped. She fell to the floor and sustained a fractured right ankle, requiring surgery.

VWR International, LLC

On August 25, 2025, at approximately 4:47 p.m., an employee was manually raising a malfunctioning loading dock door when his right middle finger was caught in a pinch point, resulting in a fingertip amputation.

An employee was replacing the heating element on a small steam sterilizer when they were burned by hot water and steam on their arms, chest, abdomen, and thighs.

A STERIS installer had uncrated a drying cabinet and was working with a STERIS installation specialist to tilt the cabinet up. As the unit was being tilted up, it slid forward, causing both employees to lose balance. The drying cabinet fell on the installer s right middle finger, resulting in an open fracture and amputation of the distal fingertip.

Sarasota Medical Products, Inc.

An employee was operating an automated bagging and packaging machine to bag hydrocolloids. A bag was stuck in the machine. As the employee was removing it, the blade amputated the top of their right index finger.

ABC Supply Company

An employee was delivering materials. He was doing his pre-delivery inspection on the roof of the jobsite (a warehouse) when he fell through a plexiglass skylight. He landed on his feet on the concrete floor 14 feet below. The employee sustained fractures to his spine, left ulnar/radius, and right heel. The employee required surgery.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

Circle Graphics, Inc.

An employee was removing packaging from a roll of printing substrate. The blade of their utility knife got stuck. The employee used both hands to free the knife and the blade partially amputated their left little fingertip.

FLATIRON CONSTRUCTION CORP.

A concrete batch plant operator was assisting with clearing spoil piles using a skid steer. The skid steer backed into a stationary screen plant. The employee's left little finger was crushed between the controls of the skid steer and the screen plant, resulting in a fracture and laceration. The employee's finger was surgically amputated.

Takkion OPS Management

An employee was standing on an extension ladder, using a torquing tool to remove bolts that secured blades to a rotor. When the torquing tool activated, its reaction arm came around and pinched the employee's right middle finger against a lifting eye. His fingertip was amputated.
