An employee was loading a charge furnace with a 3-foot by 3-foot copper cathode sheet weighing 300 pounds, using a crane. The overload alarm activated. As the employee tried to remove the cathode charge from the furnace with the crane, it struck the employee, who suffered burns to the lower back, left side, left arm, and left hand.

An employee was scraping slag off an alloy melting furnace pot when residual liquid nitrogen mixed with molten metal still in the furnace. The instantaneous mixing caused an explosion, which knocked the employee to the ground. His protective helmet and face shield were knocked off and he suffered second- and third-degree burns to the neck and left arm.

An employee was helping a millwright install the bottom block of the wire-pulling device on a wire-drawing machine. The employee was holding onto the 400-600 pound block, trying to straighten it so it would continue traveling down its shaft. The block unjammed and traveled 4-5 inches, injuring the employee's left middle finger in the nail area. The injury resulted in an amputation above the first knuckle.

An employee was changing spools on a wire drawing machine when his right thumb was caught between the lift structure and a spacer block, amputating the thumb tip (no bone loss).

Employees were assembling and testing a butterfly valve unit and actuator combo. When the unit was function tested with air, the valve gate closed on an employee's left thumb resulting in a fracture and laceration.

An employee was loading an empty reel on the west reel take-up when they noticed that the wire lying on the east reel take-up had over-accumulated on one side of the reel, making it uneven. The employee stopped the east take-up machine and released tension on the wire to correct the lay. As the employee engaged a mechanical rotation function to take up the wire slack, their right little finger was crushed between two separate wire strands on the reel under tension, resulting in a fingertip amputation. The machine was guarded at the time of the incident.

An employee was operating an overhead crane to transport a bundle of copper tubing. He went to lower the load as he was approaching a rack, but the suspended bundle of tubing suddenly jerked in the other direction. The bundle struck a nearby tool cabinet, and the cabinet tipped over. The employee went to prevent it from falling. However, the cabinet still fell over, striking his right arm and briefly pinning it against another object. The employee suffered a fractured right forearm that required hospitalization.

An employee was working to manually clear copper tubes from the turnover rack due to a faulty sensor. As he used his right hand to remove a finished tube from the turnover rack, another tube pushed through the feed line and struck his gloved right ring fingertip. The sharp edge of the copper tube caused a soft tissue amputation to the bottom side of the fingertip.

The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
