An employee was operating a powered industrial vehicle to move a pallet when their leg became caught between a bollard and the vehicle. The employee sustained a leg fracture.
An employee was using an electric pallet jack to move a rack with long metal poles. One side of the rack lifted higher than the other and the poles slid and struck his right foot, resulting in fractures to his right metatarsals that required surgery.
An employee was performing dry cleaning operations on a food forming machine when their left middle finger was struck by a plunger mechanism and they sustained a partial fingertip amputation. The machine was not guarded at the time of the incident.
