An employee was standing on a step ladder, cutting a steel pipe. The pipe fell, struck the ladder, and kicked it out. The employee fell three feet and sustained a broken left arm that required surgery.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

M1 Support Services

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

SRM Concrete

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was operating a hammer drill when his glove got tangled in the drill and his little finger was amputated.

The employee was working in the back of a micro truck and maneuvering around the materials in the back of the truck. The employee's foot caught on the edge of the truck, causing him to lose balance and fall from the back of the truck to the ground approximately 3.5 to 4 feet below. The employee landed on his feet on the loading dock but rolled his ankle. The employee was hospitalized with a compound fracture of the ankle.

An employee was assisting the operator of a soil sampling rig to collect soil samples. He went to remove soil sampling tooling and the energized equipment cycled. His right index fingertip was amputated including the fingernail.

Ironclad Environmental Solutions, Inc.

A truck driver was loading a second roll-off container onto a two-box trailer. He was operating the hydraulic control levers with his left hand, and was holding the D-ring in place on the exterior of the trailer while letting slack out of the winch cable. The employee's right little finger became pinched between the winch cable and the structure of the trailer. The employee required surgical amputation of the distal phalanx.

Montrose Environmental Group, Inc.

An employee was sampling a fluid catalytic cracking (FCC) unit by opening a valve and placing a proving tube inside to collect the sample. The sample taken is a hot gas mixture. When the sample was collected, the flange came undone and fell due to the pressure. The employee sustained an amputation to their right middle fingertip.

An employee was helping a coworker transport a tall palm tree with a mini skid steer. The employee was severely shocked by a high-voltage electrical wire above the ground.

An employee was retrieving a Christmas tree from a shelf using a ladder. He missed a step and fell to the concrete floor. He sustained injury to his head and wrist.
