An employee was putting crushed 55 gallon drums in a dumpster into a recycle trailer using a forklift. The dumpster was raised about 5.5 feet off the ground on the forks of the forklift. As the dumpster rolled forward, their left index finger was pinched between the dumpster frame and the forklift fork and the fingertip was amputated.

An employee was removing rubber that was jammed on an applicator of a tire assembly machine. As the jam was removed, the machine cycled and the employee's right hand became caught against the heated anvil on the machine, resulting in a full-thickness burn to their palm and wrist that required hospitalization and surgery.

An employee was near a backhoe while the operator was getting a broom. The broom handle struck the lever that actuates the swing of the backhoe bucket. The bucket then struck the injured employee, pinning him to the outrigger. The employee suffered crush injuries to the chest.

The injured employee was being trained on loading trucks in the load out bay. The injured employee and his supervisor climbed the stairs that run parallel to the truck and trailer, walked to the upper platform used for observation and sample taking, and saw that the lid of the grain trailer was closed. The employees began working to open the lid which appeared to be stuck due to cold weather. The injured employee pulled on one of the pivot arms that connect the lid to the main body of the trailer. As the lid began to open, the employee s right hand slipped down on the pivot arm and his index finger was caught between the pivot arm and the base of the pivot arm. The employee s right index finger was fractured and three nerves were severed. The employee was hospitalized.

An employee was working in the mixing department in an indoor warehouse when he began to sweat and cramp. He sustained heat exhaustion.

An employee was operating a rubber roller grinder and had just completed the grinding process. He changed the rotation of the spindle to the clockwise position and began to clean rubber dust off the rollers with a shop rag. The rag caught on the roller and pulled his hand into the grinding wheel, resulting in a broken wrist, broken fingers, and lacerations to the hand and fingers. He was hospitalized.

An employee was clearing jammed material from an extruder machine. The employee reached their left hand through the unguarded opening between the extruder machine and the conveyor belt to manually dislodge the jam and contacted the moving chain and sprockets, resulting in an amputation to the left thumb.

At about 10:05 a.m. on March 10, 2025, an employee was working at the loading side of a mix conveyor when his right wrist was caught in a pinch point between the conveyor and a roller. He suffered laceration and fracture injuries to the wrist and was hospitalized.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
