Final narrative
An employee was performing maintenance on a cold header machine in the MVH. He was lining up a pin hole to put a one-inch round pin between the arm of the cold header machine and a piece of metal. The arm slipped out of his hand and dropped causing a partial amputation to the employee's left index finger.
Employee reached into the press #78 and his left index finger was severed at the second joint. The press has a pop up release which is activated after the cycle, the employee placed his hand while the pop up release was in operation catching the employee's hand between the part and the top of the press.
