An employee was polishing a bar on the lathe with sandpaper when the bar wrapped around the employee's glove and pulled it off. The employee sustained fractures to their index finger, wrist, and a knuckle.

An employee was loading a baler. The baler jammed; as he tried to clear it, it crushed his right hand.

An employee was loading copper tube into a draw bench machine. The copper was pulled forward by the machine and the employee's finger was pinched between the tube and the draw bench machine resulting in amputation of the left middle finger.

An employee had just finished a maintenance function on the de-burring machine and was in the process of starting it back up. The employee was then caught between the hopper and de-burring table, suffering a crushing injury to the upper left chest, including a fractured scapula and extensive bruising. The employee was hospitalized overnight. The machine was running at the time of the incident.

An employee was helping to adjust equipment. She tripped as she was walking around the equipment. When she reached out to catch her fall, her right hand entered the point of operation. The equipment activated, pinching and amputating her right pinky finger between the stockholder tab and discharge table.

An employee was operating a band saw when his right little fingertip was amputated.

An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.

An employee was using a table saw to cut a piece of wood for wall framing. The wood jumped, causing the employee's hand to move toward the saw, amputating their left index finger and causing a laceration on their hand.

An employee was operating a trim saw. While trying to free a jammed board she had just cut, she came into contact with the running saw blade, which amputated her right thumb.

An employee was using a shaper to cut wood when their right hand contacted the blade, resulting in a partial amputation to the right ring finger.

An employee was loading an empty reel on the west reel take-up when they noticed that the wire lying on the east reel take-up had over-accumulated on one side of the reel, making it uneven. The employee stopped the east take-up machine and released tension on the wire to correct the lay. As the employee engaged a mechanical rotation function to take up the wire slack, their right little finger was crushed between two separate wire strands on the reel under tension, resulting in a fingertip amputation. The machine was guarded at the time of the incident.

An employee was operating an overhead crane to transport a bundle of copper tubing. He went to lower the load as he was approaching a rack, but the suspended bundle of tubing suddenly jerked in the other direction. The bundle struck a nearby tool cabinet, and the cabinet tipped over. The employee went to prevent it from falling. However, the cabinet still fell over, striking his right arm and briefly pinning it against another object. The employee suffered a fractured right forearm that required hospitalization.

An employee was working to manually clear copper tubes from the turnover rack due to a faulty sensor. As he used his right hand to remove a finished tube from the turnover rack, another tube pushed through the feed line and struck his gloved right ring fingertip. The sharp edge of the copper tube caused a soft tissue amputation to the bottom side of the fingertip.

An employee was cleaning slag from a casting mold at a furnace. The molten copper dropped out of the mold too quickly, reacted with the mold cooling water, and blew out. The molten metal struck his right foot, hand, and torso, causing first-, second-, and third-degree burns.

An employee was working with a drill press when his right glove was caught and pulled into the drill bit. The employee's little fingertip was partially amputated.

An employee was readjusting a safety hook to remove hardware from gang forms when his feet slipped. He fell to the ground, resulting in a broken left arm and wrist.

An employee was on a step ladder in a store aisle, making room on shelving for merchandise. She fell from the step ladder to the floor, suffering two fractures in her left leg. She was hospitalized, requiring surgery.

An employee was closing the side cargo door to a company van. The tip of his finger got caught between the door and the door jamb, resulting in a partial amputation.

An employee lost his footing while climbing an inclined ladder. He stumbled down one step and was able to stabilize himself by holding the railing and twisting his body, but he suffered an internal abdominal injury and was hospitalized.
