An employee was stacking concrete blocks that were going to be picked up by a skid steer. The employee went to move a block that was leaning against the stack. The block landed next to the stack and the employee's right hand was caught between two blocks. The employee's index finger was partially amputated and the first knuckle was fractured.

The employee was lowering a trailer hitch onto the receiver ball to connect the trailer to the truck. During the alignment, the trailer moved, causing the employee's finger to become caught between the trailer and the receiver ball. The employee's middle finger was partially amputated.

An employee was working in a basement, keeping the area secure while other employees drilled down from the floor above. The concrete core drilled out from the floor above fell down, struck the rim of the core-catching bucket, then fell about 15 feet further, striking the employee's hardhat and then the back of his head and neck.
