On September 25, 2023, a forklift maintenance technician was removing a fork from the lift truck when the fork slipped from the employee's grip. His finger was pinched between the fork and the carriage, resulting in an amputation of the right ring finger just below the nail.

An employee was operating a lift truck when it struck a stationary lift truck. His left thumb was pinched between the two lift trucks, resulting in a partial amputation.

On June 6, 2025, an employee was performing maintenance on a vertically-stored dock leveler. After completing the hydraulic repair, the employee went to exit the work area when their pant leg got caught on the bumper of the dock leveler. This caused the employee to lose their balance and fall out of the open dock door, approximately 37.5 inches. The employee sustained a lower left leg fracture.

On April 30, 2024, while at a customer location loading equipment onto a flatbed trailer for transport, an employee was injured when the trailer settled downward and pinched the employee's left foot between the equipment being loaded and the dock plate. The foot was fractured.

An employee was installing a dock leveler at a customer location. Cardboard was under the plate and a pry bar was used to lift the dock plate to remove the cardboard. The pry bar shifted and the dock plate fell on the employee's left ring and middle fingertips resulting in amputations.

An employee was fixing a mechanical casket lift in a vehicle when the lift came down on their left-hand fingers, resulting in amputation of two fingers.

Walgreens Distribution Center

An employee was helping to remove the clamp attachment paddles from a forklift so they could then repair a hydraulic leak. The clamp fell and pinned the employee's right ring finger against the mast. The employee's finger was partially amputated.

An employee was on a truck attempting to stow an aluminum extension ladder onto the truck's conveyor boom system. As the conveyor mast was lowered, the boom control area came down on top of the employee's left foot, resulting in two toe fractures.

An employee was standing on the elevated forks of a forklift to install the fork chain anchor nut. The employee slipped and as he went to catch himself, he accidentally engaged the up-down control for the forks. This caused the forks to fall onto his foot. The employee's right foot was crushed and all of his right toes were amputated.

An employee was working on a scissor lift when a portion of the unit came down and struck their head at the lift base. The employee sustained a head laceration and a skull fracture.

An employee was using a punch and hammer to remove a pin from the mast of a forklift. The mast was secured by an overhead crane. The mast shifted, and the employee's thumb was pinched between the mast and the punch, resulting in an amputation to the tip of his left thumb.

On September 22, 2025, an employee tripped and fell over a permanent fixture (angle iron) that serves as a parking block in the back of a facility. He suffered a fractured right hip and was hospitalized.

RODOC (Previously Known As: Midway Trailer Sales)

An employee was helping a customer with repairs on their trailer. While lowering the ramp door of the trailer, the customer dropped the door and it contacted the employee's left ring finger, resulting in a fingertip amputation.

An employee was working to lay a part in the vise of the band saw in preparation for cutting. The left vise closed, pinching his fingertips against the part. The employee's middle and ring fingertips were amputated and they sustained a laceration to the index finger requiring stitches.

On April 8, 2025, an employee entered an engine test cell to de-harness an engine and was exposed to hot water resulting in burns and hospitalization.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.
