An employee was lifting a cope mold using a mold manipulator. While the mold was suspended in the air, he reached under it to retrieve the rollover pin. The mold slipped out of the manipulator and fell onto his right hand, causing a severe degloving of the right forearm and hand. He was hospitalized.

At 11:49 p.m. on July 11, 2024, an employee was sanding/grinding a circular aluminum casting on a belt sander/grinder. The casting slipped, and the employee's left thumb came in contact with the edge of the belt. The employee suffered a partial amputation to the thumb.

An employee was helping a dross bin, which was being raised by a forklift. The lever to empty the dross bin was pulled, causing the bin to tip forward and up. The employee's left index finger was caught between the forklift forks and the bin, and the fingertip was amputated.

An employee was feeding a piece of material into a table saw. The sawblade amputated the employee's left index fingertip.

An employee was helping operate a hydraulic mold press. The press engaged, crushing the employee's left index fingertip. The employee suffered an amputation to the finger.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

An employee was unloading scrap from a metal bin. The bin slid out of the forklift and landed on the employee's right foot, causing fractures to the foot.

On August 12, 2025, an employee had just finished skimming slag from the top of a ladle of molten metal. They began to walk away while an overhead crane began moving the ladle. The latch that prevented the ladle from tipping came loose. The ladle tipped and spilled molten metal onto the floor and it burned both of the employee's feet.

On August 4, 2025, at 7:00 AM, an employee was approaching an area to put out a fire with an ABC fire extinguisher when an unknown material exploded and contacted their face. The employee sustained a laceration to the right cheek with an object penetrating the nasal cavity. The employee was hospitalized.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
