An employee was replacing a wire rope on an elevator. A pair of wire rope cutters were on the counterweight. When he started the elevator, the wire rope cutters fell on his foot, causing a fracture and laceration that required surgery.
On 11/29/2016, at 4:00 p.m., an employee was in the machine room when he saw debris on the sheave where the elevator cables wrap around. He started to clean the sheave using a screw driver.
