An employee picked up a sheet of aluminum plate with a crane to load the sheet onto a plasma cutting table. The employee climbed onto the table to remove a bow from the plate so it could be laid down flat. The plate was leaning over-center away from the employee. He put his foot on the bottom edge of the plate, at the bow in the middle of the plate, and then directed the crane toward himself. The chain hook holding the plate grab turned past 90 degrees, damaged the crane hook clasp, and pulled the plate grab handle out of the crane hook, causing the plate to fall. The employee saw the plate falling and tried to jump off the table. The plate hit him, pinching his left leg on the table and fracturing it below the knee.

An employee was shearing a piece of 1/4" thick X 4" wide piece of steel flat bar on an ironworker machine. The employee's gloved, left middle finger was caught between the flat bar and the shear resulting in a fingertip amputation with bone loss.

An employee was welding metal parts. The employee turned to the left and their work uniform came in contact with the location on the part that had just been welded. The employee's uniform ignited, causing burns to their shoulder and back.

An employee was using a small propane heater when they sustained third-degree burns to the left buttock and second-degree burns to the back and neck.

An employee was welding steel dump ramp slides. He leaned over and a previous weld caught his hoodie on fire. The employee sustained burns on his left side from the armpit to the waistline.

An employee was removing tubing caps and cleaning tubing ends with solvent. Their flame-resistant pants became soaked with the cleaning solvent and were ignited by a propane torch used for removing tubing caps. The employee was hospitalized with burns to their legs.

An employee was welding a kick bar onto the rear of a hopper on a trash truck. His clothing caught fire, resulting in second- and third-degree burns to his chest and stomach area.

An employee was welding lift arms when their shirt caught fire due to welding sparks or heat, resulting in thermal burns to their upper chest.

An employee was climbing up a fixed ladder alongside a building to inspect a roof leak. The employee fell off the ladder and landed on the ground about 15 feet below. The employee was hospitalized with fractures to both wrists.

On September 16, 2025, an employee went to pull a weld stand when he tripped and fell, landing on the floor on his right hip. The employee suffered hip and fibula fractures that required hospitalization and surgery.

An employee was accessing a tensioned hook that required a hoist mechanism to be engaged. He was unhooking the lift hook from a shelf column when the "up" button was pressed, which released the tension on the pneumatic air hoist. The employee's right index finger was caught between the hook and the column, resulting in a soft tissue amputation and fracture to the fingertip.

A crane was lifting a pole out of a zinc bath in the galvanizing department when the chain attached to the pole broke. The pole fell back into the bath and liquid zinc splashed onto an employee, who suffered third-degree burns to an arm, the abdomen, the torso, the upper left side of the chest, an ear, the neck, and a hand.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
