An employee was walking down a slope of 3/4 inch stone when he slipped on a patch of loose crushed stone, rolled his ankle, and fell to the ground. This resulted in a fractured right ankle and fibula.

An employee was installing fence posts when they stepped into a recently dug hole that was not visible due to rainfall. The employee's leg was fractured.
