An employee was punching holes in metal pole base plates using a piranha machine. A piece of metal flew into his left eye, injuring the side of his pupil. He was hospitalized for eye surgery. Eye protection was not being worn at the time of the incident.
On January 17, 2018, four employees were attempting to hand roll a metal combination part that was on the work floor. The part was approximately 184 inches long and approximately 650 pounds. The part was set down and crushed one employee's left-hand middle fingertip, resulting in an amputation.
