An employee was attempting to lift a rechargeable energy storage system weighing approximately 250 pounds onto a wall-mounted bracket. The employee was on the left side of the bracket with his hands under the storage system when it fell and crushed his left little finger, resulting in injuries that required surgery.

An employee was pushing electrical conduit through the eave of the house for solar circuits when their ladder moved, causing the employee to fall 15 to 20 feet to the ground. The employee sustained fractures to the nose, a tooth, and two thoracic vertebrae as well as an injury that required stitches on the face.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

Thompson Hardwoods, Inc.

An employee and co-worker were working to unjam and move a 100-pound piece of crossed-up cant timber. The employee lost their grip and the timber fell on their right hand, resulting in crush injuries to the fingers.

Maxion Wheels LLC

An employee was working on a cushion ring (steel). As they lifted the cushion ring with both hands to flip it up, the ring slid down and the employee's right middle fingertip was crushed between the ring and the work table. The employee was hospitalized with a fracture, and their right middle fingertip was amputated.

HEXPOL Compounding PC, Inc.

Two employees were lifting a roll of steel poly bar (50 inches and approximately 150 pounds) from ground level to a height of 4 feet at the 66-inch rubber calendar. One employee dropped the roll, resulting in the poly bar contacting the injured employee's right knee. The employee was hospitalized with fractured right patella.

The injured employee was walking in front of the property by the front door as another employee was removing debris from the roof. The injured employee was struck by a 2 x 4 that was being cleared from the roof. He was knocked unconscious and sustained brain bleeding, as well as head and facial injuries, and required stitches.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
