An employee was using a pipe groover machine to roll a groove at the end of a 10-foot-long steel pipe. Their gloved left index finger was caught between the pipe and the adjustable pipe guard, resulting in a partial fingertip amputation through the distal phalanx.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.
