An employee was performing production work on a rolling machine when their left glove got caught in the machine. The employee's left hand and wrist were crushed, leading to surgical amputation of all four fingers and part of the thumb. The machine was not guarded at the point of operation.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was mixing sauce using a sauce barrel mixer when his left little finger was amputated.

An employee was working on a production line. The employee went to retrieve parts that came out of the parts holder, and their left hand became caught between a chain and a sprocket. Three fingers on the hand were amputated.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a pad printer. The employee's finger was caught in the printer and they suffered a fingertip amputation. The part of the printer that caught the employee's finger was unguarded at the time.

3S Americas, Inc.

An installation technician for a wind turbine lift was using a chain hoist to move materials up a tower. The chain storage unit overflowed; the chain spilled and fell about 40-50 feet down the tower. It struck the injured employee in the face, causing fractures to their nose and cheekbones.

On November 25, 2024, at 9:00 a.m., two employees were moving a metal staging pallet that was located in an automation cell. The injured employee slipped and their left middle finger was crushed between the pallet and metal frame of the machine, resulting in an amputation to the finger padding.

On October 22, 2024, at approximately 10:15 a.m., an employee was forming a stand leg on a standard brake press. As they were readjusting the part, the tip of their right thumb was caught in the brake press, resulting in a partial amputation. The incident occurred during training.

An employee was working to remove a product blockage from a piece of equipment. The employee was struck by a metal bar near the deck of the pin mixer equipment and sustained blunt force trauma to the torso.

An employee was moving a rack onto a lower conveyor in the south heat treat area when their finger got caught between the part tray and the rollers, resulting in a fingertip fracture and amputation.

An employee was climbing a ladder to get on top of the breakroom. The ladder slipped, causing him to fall to the ground. The employee sustained fractures to his upper back, left foot, and ankle.

Two employees were removing a circuit breaker. The injured employee was using wrenches in the removal process. One of the wrenches contacted an adjacent door, resulting in an arc flash. The employee suffered burns to their neck, face, and ear, as well as momentary vision loss due to the flash.

An employee bent over to pick up a rubber mat from the ground. When he grabbed the corner of the mat, a rattlesnake hidden under it bit his left middle finger near the first knuckle. He was hospitalized.
